Recommendations on the need to aerate or not to aerate BI test packs prior to removal of biological indicators differ. AAMI's Good Hospital Practice: Ethylene Oxide Sterilization and Sterility Assurance states:

"The sterilizer manufacturer should be consulted about the need for aeration of test packs prior to BI removal. Regardless of the recommendations of individual manufacturers, however, the personnel carrying out testing procedures should be aware of the following general considerations:

  1. If the test packs are aerated prior to removal of BI's then the entire contents of the sterilized load must be aerated for at least an equivalent length of time. Regardless of when the BI is retrieved, the remainder of the load must be fully aerated; the duration of aeration depends on the contents for the load and on the temperature used for the aeration time.
  2. The removal of BI's from unaerated test packs must be carried out in a well-ventilated room, using appropriate EO exposure-control methods to minimize operator exposure to the steriliant.
  3. The contents of test packs should be aerated completely prior to reprocessing and reuse."

My tip: if the BI test is removed prior to aeration neoprene gloves should be worn during the handling of the test pack and its contents. Once the BI is removed the test pack components (e.g. syringe, peel pouch) and gloves incl.the used ETO cartridge should also be aerated with the load.
