Hello Mr. Ahmed!

In our hospital we have an in-house Standards Committee, responsible for the preparation and yearly updates/revisions of all the hospital-wide policy and procedures guidelines prior to the final approval of our hospital administration.
All of our CSSD Policy & Procedures submitted to the above-mentioned committee yearly for review/revision and approval.
You may ask your Quality Improvement Coordinator in your hospital for the standardized format of your policy & procedures guidelines.
As your reference; I am very much willing to spare you our CSSD policy and procedures guidelines format.
