Dear Samir,

Quality assurance of a CSSD department should be planned according to a certain standard. A Self Assessment Tool is a series of 24 forms used to assess decontamination practices in organizations as required by HSC 2000/032 Decontamination of Medical Devices (UK) may be obtained from the NHS website:
